Well, none of those things should have worked. You are dealing with bake fail, which used to be much more common in SL than it is today. Still, it can happen if your inventory is taking way too long to load as you log in. Until your inventory cache is fully updated, the servers won't complete the job of sending appearance files so that your viewer can render your av. Now, to prevent problems like this in the future, you ought to try to strengthen you Internet connection. You may have plenty of bandwidth, but the connection is either losing data or has a bottleneck that is slowing the time it takes to move data between your computer and SL's servers. If you're on wireless, get off it. Get yourself a LAN cable and connect directly to your router. Then unplug your router and modem from the power for a few minutes to clear their RAM and get a fresh grasp on an IP address. Restart your computer and log in. You should have improved performance overall, and you should not have further bake fail problems. (I hope)
