Oh, my. That hasn't happened for a while. There's a very old unresolved bug that does exactly that. I heard about it back in 2007, and I see that someone added a note a little over a year ago to say that she had run into it.
I really don't know that the right answer is, but maybe it has something to do with the problems that some people have with HTTP texture fetching. That's just a guess, but it wouldn't hurt to try the solutions at http://wiki.phoenixviewer.com/http_fetching_issues
