How do you fix attachments floating In mid air when warn?

Because when you attach something to a new attachment point for the first time, it doesn't know where you want it to go, so it just picks the default location and rotation for that attachment point.  All you need to do is grab the item with your Build/Edit tool and move and rotate it to where you want it.  Once you have done that and saved the item to inventory, it will always remember what you did, and will attach correctly each time.
