Is it you? Someone on your Friends list? A tag over someone's head? Usually the answer is that you are not fully connected to SL's servers. As you log in or change sims, the servers in the new sim attempt to grant you "capabilities", which means basically that your viewer may access various databases for information about friends, groups, inventory, etc. You may get an error message (and often a crash) if the attempt fails completely, but you might also just find that some services don't work. You can try teleporting to a different sim and back or relogging to see if that helps. If you're on wireless, get rid of it and use a direct LAN cable. You might also need to amend your router's list of DNS servers by adding the Google DNS servers or others from this list.
