Linden Lab has been doing a lot of maintenance on the web profile site recently.  People have noticed several odd side effects.  This may be one of them.  I suggest filing a support ticket under the category Account Issue to call their attention to your problem.  Depending on the out come of that request, you may also want to file a JIRA report.

There's no evidence that it was ever changed from this since then.

I'm not sure this helps you, but we can only diagnose what we can see, as residents.

Hehe I haven't looked in-world, but I find going the web method is more successful (also since LL can't code a website, I need manual control of the elements so can't use the in-world browser )

 

But yeah, there's no evidence of the OP's name being changed since 4 months ago.
